POSITION SUMMARYThe Licensed Vocational Nurse (LVN) delivers comprehensive nursing care to medical /surgical patients on an assigned unit under the direction of a Registered Nurse (RN). Patient care duties are performed in a safe and proficient manner according to individual patient needs. He/she provides direct patient care based on Hospital Standards of Care and Standards of Practice, Policies, Procedures and Practice Guidelines. Provides guidance and assistance to Nursing Techs, Nursing Assistants and Unit Secretaries.
Accepts assignments on other Medical/Surgical units when necessary. Acts as patient advocate within the hospital environment.************************************************************************************ NOTE: The essential job functions of this position are preceded by an asterisk (*).************************************************************************************PRINCIPAL RESPONSIBILITIES*1) Contributes to assessments performed by RNâs by documenting complete and thorough data collection at the time of patientâs needs.*2) Plans for care, including discharge plans, are reviewed daily and revised as patient conditions and needs change.*3) Documentation is consistent, complete, thorough and legible. Documentation includes the outcome of nursing interventions and patient responses.*4) Involves the patient and/or significant other(s) in the delivery of patient care. Education/instruction is provided specific to the patientâs health care needs and age requirements.*5) Administers prescribed medications to patients in accordance with Hospital policies and practice guidelines.*6) Physician orders for all assigned patients are reviewed as new orders are written, transcribed accurately, dated, timed signed and noted.*7) Physician orders are carried out accurately in providing direct patient care.
Assures that other departments are notified of orders and services are needed.*8) Accepts only physicianâs verbal or telephone orders in accordance with practice guidelines. Orders are written accurately to include date, time and Physician giving order.*9) Notifies the Charge Nurse and/or physician of changes of patient status as appropriate.*10) Considers age-related factors/limitations in the education of patients. Applies knowledge and principles of age-related factors appropriately in teaching situations.*11) Ensures patientâs right to privacy and confidentiality.*12) Assumes responsibility for the nursing care of assigned patients. Patient care activities are organized and prioritized.*13) Patient situations requiring care beyond the scope or practice of an LVN are brought to the attention of the Charge Nurse or RN.*14) Contributes to the smooth functioning of the Unit by offering help to other staff members.
Assists with patient care functions beyond those assigned.*15) Provides accurate and pertinent patient information reports too the Charge Nurse and to the oncoming shift to ensure continuity of care.*16) Performs Unit-specific responsibilities such as narcotic counts, crash cart checks and cleaning of equipment and work areas*17) Administers and maintains IV fluids and IV medications in accordance with Hospital practice guidelines. Correctly labels, calculates and regulates the flow of IV solutions. Documents pertinent information related to IV therapies.*18) Prepares patients for surgical and other invasive procedures.*19) Performs routine post-op care in accordance with Hospital procedure.*20) Maintains competencies required by Hospital, Unit and position.*21) Demonstrates safe, effective working knowledge of equipment.*22) Maintains a working knowledge of the crash cart, emergency equipment and code procedures.*23) Performs basic life support (CPR) when necessary.*24) Takes into consideration the spiritual, psycho-social, cultural and religious preferences of the patient and family when participating in care.*25) Performs other tasks/duties as assigned or requested. Assumes responsibilities for the care of all assigned patients.
Patient care activities are organized and prioritized.*26) Demonstrate understanding of HCAâs and "Patients First" safety initiative by strict compliance to all safety protocols and procedures.ADDITIONAL JOB REQUIREMENTSEDUCATION, EXPERIENCE, CERTIFICATIONS, or REGISTRATIONS: Recent critical care experience preferred. Additional education towards completion of an accredited professional nursing (RN) education and ACLS preferred.ADDITIONAL JOB RESPONSIBILITIES*1) Meets the requirements of the LVN job description as appropriate.*2) Demonstrates safe and effective working knowledge of all equipment used in the critical care units as appropriate.*3) Functions as directed by the RN in Code/Arrests.*4) Considers age related factors in the assessment, care, and treatment of the infant, pediatric, adolescent, adult, and geriatric patient.*5) Provides care for the critically ill patient with central/invasive lines.*6) Under the guidance/supervision of the RN is able to provide care for critical ill patients with: a. Hemodynamic pressure lines: PA lines, CVP lines, arterial lines b. Pressure monitoring devices: ICP c.
Temporary pacing d. C.O./C.I./C.C.O. e. Artificial airway/mechanical ventilation*7) Provides care for the patient on or receiving vasoactive/potent drips or drugs (within the constraints of hospital policy for LVNs).*8) Provides care for the unstable cardiac patient and is familiar with EKG/rhythm strip interpretation.*9) Provides care for the immediate post operative patient.*10) Applies the principles of critical care nursing to the critically ill medical or surgical patient: orthopedic, cardio-vascular, neuro, pulmonary, endocrine, hematological, GI, GU, and renal).
Able to intervene appropriately in a critical or crisis situation.*11) Considers principles of death and dying when caring for the caring for the terminally ill medical/surgical patient. Provides emotional, spiritual, religious and cultural support to patients and their families.*12) Demonstrate understanding of HCAâs and "Patients First" safety initiative by strict compliance to all safety protocols and procedures.Requirements: LVN licensure in the State of TexasPreferred: One year experience in the acute care setting
