Job Description How would you summarize my contribution to THERAPY 2000?The Therapist I, II, III position is a qualified, licensed professional who provides therapy services to patients in their home as prescribed by the atte! nding physician. Patient visits are predominately performed in private residences or childcare settings. Each Therapist is the company's frontline for improving the lives of children challenged by disabilities or developmental delays. This individual is responsible for each patient and their family's quality of care, customer service experience, and sense of confidence in the company's services. A Therapist is responsible for coordinating THERAPY 2000's resources to meet each family's overall health needs.How do I develop expertise in my position? The Therapist position is divided into three levels of expertise. Each level is tied to an increase in independence, impact and success.Level 1 This level reflects a Therapist new to THERAPY 2000. This individual is still developing the required knowledge, skills and behaviors to meet the expectations for the position. Areas of need might range from basic clinical experience to pediatric-specific skills to T2K policies and procedu! res. As a result, this individual demonstrates limited indepen! dence and is required to seek guidance when fulfilling day-to-day responsibilities. Level 2 This level reflects the requisite knowledge, skills and behaviors to meet expectations in the Therapist position. This individual demonstrates the necessary independence, critical thinking and decision-making to carry out the position's essential job duties with limited support from his/her supervisor. Individuals at this level are eligible to provide clinical supervision within their discipline's guidelines and pursue additional specialization. Level 3 This level reflects a tenured Therapist that has demonstrated the ability to exceed company expectations in areas of clinical specialization. As a result, this individual might receive additional opportunities to contribute to the company's success. Completes a minimum number of visits defined by the agency.2. Performs in depth patient evaluation and develops treatment plans in accordance with physician's orders.3. Implements treatment plan, provides evidence-based, best practice therapy services.4. Instructs patients, their families and other members of the health team in regards to therapy services.5. Reports pertinent observations and reactions regarding changes in patient status to the appropriate team person.6. Maintains patient's medical record in accordance with agency standards, completes and submits patient records in a timely manner.7. Participates in patient care conferences, communicates with other members of the home health team to facilitate coordination of patient care.8. Participates in! staff development and in-service education with the agency to learn new treatments, procedures, developmental skills etc.9. Performs all duties in compliance with agency policies and procedures. Adheres to all rules, regulations, and code of ethics, guidelines and codes in federal, state and local laws, which are concerned with the practice of his/her therapy discipline and home health setting.10. May be required to meet with supervisor, technical support personnel, staffing personnel, or other support staff as needed to maintain therapy standards and patient care.11. Incidental time is a part of the job responsibilities which includes, but not limited to, patient chart updates, patient notes and call logs, performance reviews, supervisory meetings, and drive time to patients home.12. Must be physically and legally able to operate a vehicle in the State of Texas.What are some examples of preferred experience and skills?1. 1-2 years of pediatric services2. 1-2 years of home health experience.3.
