The Pediatric Hematology/Oncology (H/O) Clinical Pharmacist Specialist provides comprehensive pharmaceutical care to ! pediatric H/O patients in the Children’s Hospital through integrated drug distribution, clinical services, research and teaching.

The major job responsibilities of this position are: pharmaceutical care to pediatric H/O patients, drug distribution support, clinical intervention, investigational drug studies, research, teaching and pharmacy representation on pediatric committees.

The following activities are components of the position to assist in meeting the major job responsibilities:

1.

Provide comprehensive pharmaceutical care to pediatric H/O patients.

a.

Provide clinical pharmacy support for patients of the pediatric H/O service.

b.

Discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therape! utic duplication, and drug interactions.

c.

! Provide pharmacokinetic consultations as required.

d.

Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.

e.

Participate in the drug therapy management of medical emergencies.

f.

Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary.

g.

Facilitate appropriate ordering and preparation of TPN by identifying and resolving inappropriate orders (i.e. calcium, phosphate, osmolarity, amino acid concentration, fluid volume, etc.)

h.

Maintain and update a patient profile with allergies, and current medications.

2.

Facilitate discharge/transfer for pediatric H/O patients.

a.

Anticipate discharge/transfer date and medications.

b.

Counsel/triage patients/caregivers on discharge/transfer medication! s as necessary

c.

Facilitate discharge/transfer planning and smooth transition to alternate site of care or the outpatient pharmacy.

d.

Assess insurance drug coverage well in advance of discharge/transfer and assist in preauthorization for medications, as necessary.

3.

Specific pediatric H/O functions:
a.

In collaboration with Medical Staff, responsible for development of chemotherapy orders, to include, calculating body surface areas, calculating all patient specific doses, calculating needed compatible fluids for admixture, writing for pre and post chemotherapy hydration and adjunct medications.

b.

Responsible for providing clinical support for pediatric H/O clinic and Children’s Hospital inpatient nursing staff regarding chemotherapy medication administration and related questions.

c.

Responsible for reading applicable protocol-specific treatment plans, medicat! ion administration guidelines, medication dosing adjustments, required ! labs as they pertain to appropriate dosing, and toxicity monitoring and adjusting subsequent dosing.

d.

Responsible for processing chemotherapy medication orders in a computerized patient files for

the pediatric H/O service.

e.

Responsible for ensuring chemotherapy, adjunct medications, and needed supplies for the pediatric H/O patients are readily available.

f.

Responsible for maintaining chemotherapy medication files in the pharmacy with a copy of chemotherapy

Responsible for assisting with pain management of pediatric H/O patients.

h.

Responsible for providing medication consults to pediatric H/O Medical Staff, nurse practitioner, and nursing staff.

4.

Provide operational support in the Children’s Hospital Pharmacy as determined by departmental need.

a.

Review medication orders for accuracy and appropriateness.

b.

Order entry f! or assigned areas.

c.

Resolves drug distribution related issues.

d.

Resolves Pyxis related issues.

e.

Covers assigned shifts as determined by departmental need.

5.

Promote and demonstrate rational, cost-effective drug therapy.

a.

Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved.

The ultimate goal is to develop and assure cost effective clinical pathways.

b.

Develop and conduct target drug programs and medication usage evaluations as needed and report results to the Pharmacy and Therapeutics Committee.

c.

Document clinical interventions and cost avoidance as part of the departmental documentation program.

d.

Monitor the use of medications to assure there is use consistent with approved criteria.

e.

Anticipate changes in drug therapy and report the potential impact! it will represent to the overall hospital and drug budget.

f.!

Monitor treatment costs and associated reimbursement and provide financial forecasts to the Department of Pharmacy Services.

6.

Facilitate investigational drug studies.

a.

Coordinate the pharmaceutical care of ongoing research in pediatrics.

b.

Provide in-services for pediatrics related investigational studies to the nurses and pharmacy staff.

c.

Develop methods and systems to reduce the risk for medication errors in investigational drug studies

d.

Anticipate the pharmacoeconomic impact of investigational drug studies. Provide financial forecasts to the Department of Pharmacy Services.

7.

Take a leadership role in the teaching programs of Children’s Hospital Pharmacy Services and

College

of

Pharmacy

as they are related to acute care services for pediatrics.

a.

Attain and maintain a

Pharmacy Adjunct

appointment.

b.

Precept and mentor pharmacy practice and pediatric specialty residents.

c.

Precept Pharm.D. clerkship students.

d.

Provide staff development support to nurses and physicians.

e.

Provide didactic lectures in pediatrics at the

College

of

Pharmacy

and/or

College

of

Nursing

.

f.

Facilitate the personnel development in the area of pediatrics for pharmacists.

g.

Provide presentations, publications, and other informative activities in the area of pediatrics and other drug-related topics to the health care community and general public.

8.

Participate on

Medical

Center

committees.

a.

Participate actively in team meetings, which include pediatric collaborative, discharge management, scie! ntific research, and educational teaching conferences.

9.

Performs other duties as assigned.

IV.

Minimum Position Requirements:
Work Experience:
Two years work in experience in pediatric hematology/oncology, of which one year should be in

Pharmacy Practice Residency (PGY1) and one year in Pediatric Specialty Residency (PGY2), or equivalent pediatric hematology/oncology experience.

License/Registration/Certification:
Licensed Preceptor in

Texas

Licensed (or license eligible) as a Pharmacist in the State of

Texas

Certification in Pharmacotherapy is preferred.

If certified, maintains a certification minimum of 15 hours continuing education annually.

Education and Training

:

Graduation from accredited

School

of

Pharmacy

.

12 hours of ACPE approved continuing education annually,

Maintains competency in unique aspects of drug therapy for pe! diatric patients.
