Nurse Practitioner - Resource/PRN

Job Summary

Under the supervision of the Medical Director or other staff physicians, the Nurse Practitioner will partner with both the Hospice and Palliative care team to direct and facilitate the care and support of patients, families and the physicians who treat patients that are facing chronic illnesses or a life threatening illness. The Nurse Practitioner will join and assist the Physician in providing comprehensive medical care to patients in a variety of settings, including: home, hospitals, nursing homes, and assisted living facilities. As a member of the Palliative Care team the Nurse Practitioner will provide education on Hospice care for physicians, patients/! families and others.

Responsibilities

1. Dev! elop and implement clinical plans of care for adult patients with chronic and terminal illnesses.

2. Perform a wide range of activities, which include: assessment, history and physical; recommendations on aggressive symptom management;

3. Develop plans to meet patient and family physical and psychosocial needs;

4. Monitor patient progress; update the Team Director and team members; consult around discharge planning; and patient/family teaching.

5. Maintain patient medical records according to established standards and documents in a systematic, accurate and succinct format.

6. Maintain communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis.

7. Refer patients to Physician or to specialized healthcare resources for consultation or treatment as appropriate.

8. Initiate communication with attending physicians, other hospice staff mem! bers and other agencies as needed to coordinate optimal care and use of resources for the patient/family.

9. Provide patient/family education and counseling including instructing the family and/or patient caregiver on:

How to administer medication and recognize side effects.

How to perform personal care and oral hygiene.

How to assist in lifting, moving, ambulation and exercise.

How to prepare for the events preceding death.

10. May provide appropriate support at time of death.

11. Assist in making home health aide assignments, prepare written instructions for the aide and supervise the aide in the home.

12. Participate in continuing education and/or continuing medical education as required by licensing body.

13. Prescribe medication as permitted by the state and/or the state Board of Nursing.

14. Serve as a resource to hospital, facility staff and/or physicians on palliative care issu! es.

15. Participate in hospice orientation and ongoing educati! on programs.

16. Other related duties as deemed necessary by the employer.

Qualifications

1. Nurse Practitioner licensed in state(s) practicing.

2. Certified by a nationally recognized advanced nursing practice organization.

3. Current Cardio Pulmonary Resuscitation (CPR) certification.

4. Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.

5. Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.

6. Demonstrated expertise in pain/symptom management and advanced care planning.

7. Demonstrated commitment to hospice philosophy of care and willingness to serve on-call.

8. Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance.
